Fascia and Pain (Carla Stecco)

In recent years the innervation of the fascia and soft tissues has become increasingly important in the etiopathogenesis of pain, but actually the relative importance of the various anatomical structures is not known. In this presentation the type and quantity of innervation of the various soft tissues will be presented. Besides, we’ll discuss the different modalities that can explain fascial pain:

1. An alteration of the fascial tension: in this case the fascia maintain the correct microscopic
and macroscopic structure, but it perceives an altered tension
2. An alteration of the macroscopic structure of the fascia, as a scar tissue
3. An alteration of the microscopic structure of the fascia, and here we’ll listen all the
microscopic alteration that we know until now:
a. Alteration of the density of innervation
b. Alteration of hyaluronan amount and hydration
c. Alteration of the collagen and elastic components
d. Alteration of the hormone receptor

Only knowing all these variabilities, we can approach a fascial pain in the proper way and we can understand way patients answer to our treatments differently.
